    Prepare for your appointment 


    Dear New Patient,

    Thank you for choosing Baton Rouge Cardiology Center. It is our desire to provide you with excellent and timely service. The following guidelines were designed to facilitate this process.

    Before Your Appointment

    Please check with your insurance company to see if you are covered for services at Baton Rouge Cardiology Center. When you come for your appointment, be prepared to pay any co-payments identified by your insurance plan.

    On the Day of Your First Appointment - CHECKLIST:

    • Please arrive 15-20 minutes prior to your scheduled appointment time.
    • Bring your current insurance card to every appointment.
    • Photo identification, such as a driver's license or state photo ID
    • Be prepared to pay your co-payment at every visit. Your co-payment amount is normally indicated on your insurance card.  For your convenience our office accepts checks, money orders, and all major credit cards.
    • Your employer's name and phone number- if you are covered by insurance or managed care (HMO or PPO) provided by that employer
    • Your spouse's employer's name and phone number- if you are covered by insurance or managed care (HMO or PPO) provided by your spouse's employer
    • Medicare or Medicaid card, if that applies to you.
    • Medical records, X-Rays, lab test results. Your physician may have sent these to us already - be sure to ask your doctor before your visit here.
    • All current medications or a list of current medications, how much of each you take, and how often you take each
    • Name, address, and phone number of your referring physician
    • Health Plan Referral Authorization—check with your primary care physician to see if a referral is required.
